Case manager/Registered Nurse
- Christchurch - Hereford Centre (Specialist Mental Health)
- Permanent, part-time (.8 FTE) Position

About the role
We are pleased to offer an exciting opportunity for an experienced Registered Nurse to join the Assertive Outreach Team at the Hereford Centre.
The Hereford Centre provides specialist mental health support to individuals experiencing complex, severe, and enduring mental health challenges. Our Assertive Outreach approach works specifically with people who have historically found it difficult to engage with conventional community mental health services and who may be at increased risk of relapse or admission to inpatient care.
The team is currently otherwise fully staffed and holds a high level of clinical expertise. We are seeking a motivated and positive Registered Nurse to contribute strong clinical skills and work collaboratively within a supportive multidisciplinary team.
About You
We are looking for a professional and highly motivated nurse to join our team. To be successful in our team you will need to display a positive attitude and enthusiasm for this area of work, you will enjoy being part of a committed and diverse multi-disciplinary team, you will work in collaboration with a group of professionals that recognises the importance of providing quality care and services to their patients, friends and whānau. Interacting with a variety of stakeholders, you will have an ability to effectively manage and prioritise a caseload in a case management position.
- You will work within a multi-disciplinary team, and in collaboration with our tāngata whaiora and their family/whanau, primary care, community supports and our inpatient teams.
- It is essential that you have the ability to think on your feet, triage and prioritise your tāngata whaiora's needs appropriately, and in a timely manner.
- You will case manage a caseload of tāngata whaiora who you will proactively engage with and support to engage with their treatment and other holistic supports.
- Highly developed risk assessment is important as is your ability to develop effective rapport with people who may be mentally unwell or distressed.
- Assessment, development of treatment plans and liaising with whanau and other carers, NGOs, police, probation and other services is all part of the Community Mental Health Nurse role.
You will have
Essential:
- Registered with the Nursing Council of New Zealand with a current APC and mental health scope
- Ideally, substantial experience working within community mental health and a sound understanding of case management within this context.
- Have a full New Zealand driver's license
- Knowledge of and a commitment to Te Tiriti o Waitangi
- Excellent interpersonal skills and relationship building
- Thrives in a fast-paced environment in a varied role
- To be successful in this role, you will need to bring substantial experience
Desired:
- Have a clinical background in addressing substance abuse.
- Duly Authorised Officer (DAO) status would be a significant advantage. If the successful candidate is not a DAO then we would expect that they work towards picking up the role within the first year with the Hereford Centre.
- Working alongside the equally well initiative the successful candidate will prioritise the physical wellbeing of our whaiora and either bring the skill or be willing to learn taking ECGs, venepuncture and general physical health recordings.
