Community Mental Health Nurse - Forensic

About the role

As part of The Forensic Community Team (FCT), based at Hillmorton Hospital, you will provide Case Management for tangata whaiora within a Community setting.

We are a small, multidisciplinary team who are big on supporting each other, and we follow an assertive outreach model of care – you will get to know not only the patients in your own caseload, but also those under the team’s care. 

About You

This role involves managing and prioritising a caseload, where you oversee all aspects of care for consumers. This includes assessments, home visits, follow up appointments, and organising reviews within your multidisciplinary team

Your ability to build and maintain therapeutic relationships with consumers, family/whanau, the team and a range of NGO agencies in the community will find you developing consumer focused care plans. Strong communication skills will further support this.

You’ll work with a high level of autonomy and think creatively/outside the square to support our consumers.

This is a busy role, so you’ll need to be well organised, yet flexible enough to work within an ever-changing environment. Strong problem-solving skills will help you proactively address crises as they arise.

Skills & Experience

  • An experienced Registered Nurse with a recent mental health background and a current NZ practicing certificate.
  • Someone who is enthusiastic, motivated, with excellent interpersonal skills and the ability to communicate effectively with family/whanau, colleagues and significant others.
  • A confident practitioner with demonstrated clinical assessment skills.
  • Previous experience in working with consumers who have an intellectual disability would be an advantage, although if you don't have this, we are still interested to hear from anyone who has an interest to work and gain experience in ID.
