We are looking for a compassionate and highly organised Coordinator to join our Home & Community Support Services team. This role is central to ensuring coordinated, person-centred care that supports consumers to live well, age well, and experience a respectful end of life within their own communities.
About the Role
As the Coordinator, you will work closely with referrers, nursing teams, and support workers to coordinate care services that meet the needs of clients and their whānau. You’ll play a vital role in ensuring services are delivered seamlessly, respectfully, and in a culturally safe manner.
As part of the Administration and Clerical Pay Equity Claim settlement this role has been mapped to National Band 4 /Profile 4

Key Responsibilities
- Coordinate service requests in collaboration with Service Referrers and Coasters Nurses
- Liaise with Support Workers and clinical leads to manage rostering, including leave and coverage
- Deliver high-quality, responsive customer service to clients, whānau, and families
- Support care that is culturally safe, respectful, and upholds the dignity and mana of each client
- Ensure clients are actively involved in planning and decisions about their care
- Follow all organisational policies and procedures
- Use systems and technology to support effective administration and communication
- Participate in ongoing training and professional development
- Support onboarding and skill development of new team members
- Undertake additional duties as agreed with your manager
About You
Essential:
- Strong customer service skills with a caring and empathetic approach
- Confident using Microsoft Office and similar systems
- Excellent communication and organisational skills
Desirable:
- Previous experience in rostering systems
- Previous experience in a service coordination role
- Experience in health, community, or aged care settings
