Occupational Therapist - CREST

About the Role
The Community Rehabilitation Enablement Support Team (CREST) is at the forefront of community-based healthcare, working with clients to optimise safety and independent functioning in their own homes. As an Occupational Therapist in CREST you will be required to look at clients holistically but focus on the details to meet their safety and rehabilitation needs at home.
We have a full time position(1.0FTE) opportunity, 40 hours per week for a NZ Registered Occupational Therapist.
In this role, you will be responsible for:
- Undertaking occupational therapy assessments and developing and implementing intervention and treatment plans in conjunction with the client and whānau
- Managing and prioritising a busy caseload
- Carrying out assessments and treatments plans in a holistic manner with a view to possible referral to other disciplines and support agencies
- Troubleshooting barriers to discharge
- Delegation to kaiāwhina/ AHA to ensure client needs are met in a timely and safe manner
- Ensuring timely assessment of patient needs on discharge to ensure optimum support is provided as they return to their homes
About You
Ideally you will be an Occupational Therapist who is experienced in gerontology and/or rehabilitation, who is excited to work in a patient outcomes environment.
Your skills will include:
- The ability to hit the ground running with the energy and passion to deliver a high standard of care to patients
- Flexibility and adaptability to work as a member of an inter-disciplinary team
- Enthusiasm for knowledge sharing across and beyond your discipline
- The ability to establish an early rapport with patients and families, and interact with a wide range of people from varying backgrounds
- Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth
- Good clerical, literacy and computer skills
- Demonstrated specialty area experience and professional development is preferable
- As this is a community position and travel is required, a full NZ Driver’s license is essential
- Registration with the NZ Occupational Therapy Board and an Annual Practicing Certificate is essential
