Occupational Therapist – Home Visit Team

About the Role

We have an exciting opportunity to join the supportive Occupational Therapy team at Christchurch Hospital! We are looking for a suitably experienced NZ Registered Occupational Therapist to join us as part of our Community Home Visit team on a fixed term basis, as parental cover until 2nd May 2027.

In this role you will provide Occupational Therapy assessment and therapeutic interventions for people in their own homes following a recent admission to Christchurch Hospital. This is a part-time role at 0.4FTE (16 hours per week), working Monday to Friday.  It is also expected that you will contribute to the weekend roster for the in-patient wards.

In this role, you will be responsible for:

  • Working with patients in their own homes following a recent acute admission to Christchurch Hospital, to support their discharge back to community living.
  • Be providing pre-discharge home visit assessments as clinically required.
  • Supporting your colleagues across the team with complex discharge planning 
  • Working with patients from diverse social and cultural backgrounds and working collaboratively with whanau, the wider multidisciplinary team to ensure the best outcomes;
  • Identifying and contributing towards service improvement initiatives.
  • Being involved in transfer and discharge planning.
  • Mentoring and supervision of other colleagues within the wider Occupational Therapy service
  • As part of the team you will also contribute to the weekend roster at Christchurch Hospital
  • There will also be the opportunity to upskill in acute in-patient working as needed/wanted

About You

Please note this role is open only to current staff of Health New Zealand. Please do not apply if you are not currently employed by Health New Zealand.

You will have a minimum of 2 years relevant clinical experience with sound clinical skills in occupational performance assessment and problem-solving skills. Experience working in the community would be an advantage but is not essential. 

  • Comprehensive clinical assessment and problem-solving skills,
  • Ability to work autonomously and collaboratively, to develop positive interpersonal relationships, and work flexibly within the inter-disciplinary team, with a team player attitude
  • Excellent communication and interpersonal skills, to liaise effectively with health professionals both within the in-patient services and in the community
  • Strong clinical reasoning skills and use of evidence informed practice
  • A sound knowledge of the role that adaptive equipment and environment modifications can play in supporting people’s occupational performance
  • A knowledge of the Enable NZ and ACC funding processes
  • Effective time management and organisational skills
  • Strong prioritisation skills and experience with complex discharge planning
  • Registration with the NZ Occupational Therapy Board and current Annual Practicing Certificate (or eligibility to obtain);
  • A positive attitude and enthusiasm for this area of work;
  • A full clean drivers licence is essential
