About the Role
Our South West Community Team within Older Person’s Health and Rehabilitation provides assessment of elderly clients within the community to understand any support services or rehabilitation needs. Our service is forward thinking and our allied health take an holistic approach to assessing the needs of the elderly in the community in conjunction with a complete interdisciplinary team. We work in a restorative model of care – working alongside the client promoting independence and assessing options for minimising risk within client-centred goals. Our work is focused on listening to our clients and helping to support and enable people and advise.
As an Occupational Therapist you will be required to look at clients holistically but focus on the details to meet their safety and rehabilitation needs at home. This role has Monday to Friday regular office hours with some flexibility around hours. In this role you will be responsible for:
- Undertaking occupational therapy assessments and developing and implementing intervention and treatment plans in conjunction with the client and whānau
- Assess, inform, coach and rehabilitate clients in their health journey
- Managing and prioritising a busy caseload
- Carrying out assessments and treatments plans in a holistic manner with a view to possible referral to other disciplines and support agencies
- Take pride in supporting clients to navigate the health system and enable great outcomes
- You will be able to work autonomously in the community while being backed by a comprehensive interdisciplinary team
- Ensuring timely assessment of patient needs on discharge to ensure optimum support is provided as they return to their homes
About You
We are seeking an experienced Occupational Therapist with the ability to hit the ground running with the energy and passion to deliver a high standard of care to patients to join our team in this permanent full time opportunity.
- Flexibility and adaptability to work as a member of an inter-disciplinary team
- Enthusiasm for knowledge sharing across and beyond your discipline
- The ability to establish an early rapport with patients and families, and interact with a wide range of people from varying backgrounds
- As this is a community position and travel is required, a full NZ Driver’s license is essential
- Registration with the NZ Occupational Therapy Board and an Annual Practicing Certificate is essential
- Demonstrated specialty area
experience and professional development is preferable
