About the role

Our Social Work team is part of a collaborative health system, where everyone works together to achieve positive health outcomes for patients, their families/whānau and communities. This role would initially be based within Older Persons Mental Health working primarily on the Burwood Day Clinic (BDC), but also providing Social Work Services to other areas of work within Older Persons Health and Rehabilitation in both our out-patient and in-patient settings as required.

The Burwood Day Clinic (BDC) is a speciality service that provides comprehensive outpatient assessment and treatment for older people with a significant mental health diagnosis or concerns requiring an inter-disciplinary team (IDT) approach.

In this role, you will be responsible for:

  • Follow protocols in accordance with the Service
  • Assessments, interventions, discharge and rehabilitation planning
  • Completing InterRAIs and providing service coordination
  • Respond to health and disability needs that impact on the wellbeing of individuals and their family/whānau
  • Maintain clear, accurate documentation in line with clinical standards and confidentiality policies
  • Undertake a range of statutory functions and responsibilities relevant to legislative requirements, this may include: child protection; mental health services and vulnerable adults
  • Provide a consultation service
  • Action social work interventions, requests for information, and providing reports within adequate timeframes

About You

You are a compassionate and skilled social worker with a minimum of 5 years experience and a strong background working with the elderly and in mental health settings. Your ability to work collaboratively within multidisciplinary teams and your commitment to professional development make you an invaluable asset. To be successful in this role your skills will also include:

  • A flexible and adaptable approach to your work
  • Prior experience with ACC
  • A robust understanding of rehabilitation
  • A robust understanding of hospital processes and procedures
  • Good understanding and experience with IPV/EAN
  • Good interpersonal skills, with ability to form relationships with clients and whānau under stress
  • Demonstrated experience in a Mental Health setting
  • A robust understanding of Risk Assessment
  • Technologically proficient
  • Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth 
  • Registration under the Social Workers Registration Act 2003 with current APC
  • 5+ years’ experience within Physical and Mental Health as a practitioner
